Dear Judge T. Jacobs:

You were the presiding judge who officiated my adoption on April 25, 2001 out of the juvenile court located at 1810 S Lewis in Mesa, AZ. My adopted parents are Kimberly and Joshua Seitz. Although I was six at the time, I do not remember my original last name. Can you help me with this? I am working on closing this chapter of my past. I am hoping that you might remember my adoption. I am now 22 years old and living in Boise, Idaho.
